What documents are required to apply to the CHCI Scholarship Program?



The following documents are required for application to the Scholarship Program 

Documents that MUST be completed online:

One-page resume listing education, extracurricular activities, honors, employment, community experience, and special skills or abilities.

One letter of recommendation.

Electronically signed copy of the Application Agreement

Completion of online application

Document that MUST be mailed into CHCI and postmarked by April 16, 2010:

Complete copy of your 2010-2011 Student Aid Report (SAR) with the Expected Family Contribution (EFC) clearly indicated.

**Please Note: Due to the large amount of mail we receive every day, we are unable to check if we have received your document. When mailing out SAR at the US Post Office, ask them about the tracking and/or confirmation options so you can personally track your document to see if it arrived at CHCI.
