What documents are required to apply to the CHCI Scholarship Program?
The following documents are required for application to the Scholarship Program
All documents are to be submitted online ONLY:
One-to-two page resume listing education, extracurricular activities, honors, employment, community experience, and special skills or abilities
One letter of recommendation
2013-2014 Student Aid Report (SAR) with Expected Family Contribution (EFC) clearly indicated on report
Electronically signed copy of the Application Agreement
Completion of online application
